(1) Business License. A business license application shall be submitted and signed by the owner or his duly authorized agent. All fields on the application must be completed for the application to be approved.

(2) License fees shall be computed on the basis of information contained in the application and any other reliable information known to the business license program designee. No person shall knowingly sign or submit a business license application containing false information.

(3) The applicant shall notify the City within 30 days of any change of any information contained in the application. The application form shall contain a notice informing the applicant of this requirement. Failure to notify the City shall be treated as a misrepresentation.

(4) The Chief Financial Officer or any designee may examine during normal business hours the books, papers and other records of any person to determine the accuracy of such information.
